How do I setup an appointment?

What should I bring to my first appointment?

  • Photo Identification.
  • If you have dental coverage,  information is required for the primary subscriber :   employee #,  date of birth  social security # and Insurance Company name and address
  • Patient Registration Forms (Click Here)
  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

How long has Westside Dental Works been in practice?

Dr. Richard Grossman and Westside Dental Works have been in practice since 1978.

Do you examine children at your clinic?

Are my personal files and information kept confidential?

West Side Dental Works takes your privacy very seriously. We comply with all HIPPA laws.

What types of insurance are accepted?

We accept most major dental insurances.  Please call the office for a current list. This is subject to change. Please bring your insurance information with you to the consultation so that we can expedite reimbursement. Contact us with questions regarding our participation in any insurance plan.

 

Do you have parking facilities?

Yes.

Free Parking is available in front of the building, and also available along North Atherton Avenue.
